I recently addressed the reason why weight loss drugs have historically fared quite badly, and in my view are likely to do so for the foreseeable future. Weight gain is normal when calories in exceed calories out on a daily basis. You cannot medicate away normal human physiology — any more than you give a fish a pill to let it breathe out of water.

Now, I would like to pick up the tale where I left off. Because there is more to the story of human weight gain than calories in versus calories out, as not a few of you have stressed in your comments and correspondence. Fundamentally, we are all a lot alike, but also quite a bit different.

The ways we are alike are what I have emphasized so far. It is, indeed, normal for the human body — any human body — to turn surplus calories first into glycogen (a small carbohydrate energy reserve) and then into fat (a much larger potential energy reserve).

And we are all governed by the prevailing laws of physics that relate matter and energy. Calories are a measure of energy, and matter cannot be created without energy input. Arguments against the fundamental role of energy balance in weight regulation — against calories in versus calories out — are arguments with Isaac Newton. Folks, nobody wins an argument with Isaac Newton!

And yet, I believe the innumerable patients I've had over the years who tell me something doesn't quite add up for them. I believe them when they tell me they eat and exercise the same amount as someone else — but they get fat, while the other person stays slim. Or, worse still, that happens even though they eat less, and exercise more. If they are telling the truth- and I am thoroughly convinced they are- doesn't it mean Newton had it wrong?

Nothing of the sort. All it means is that we are different, as well as alike.

While it is true for all of us that weight control is overwhelmingly, if not entirely, about calories in versus calories out, it is equally true that the number of calories required for weight loss, weight maintenance and weight gain vary drastically among us. And we even know why, for the most part.

There are, once we are done growing up, three ways we burn calories: physical activity, the generation of heat and just existing. There are technical terms for the second and third: thermogenesis, and resting energy expenditure (sometimes referred to as basal metabolic rate). What should be noteworthy right away is that you are not in charge of two out of the three!

You can choose how much exercise to do. But you don't get to choose how thermogenic you tend to be, and that can matter quite a lot. Like exercise, thermogenesis accounts for roughly 15 percent of total energy expenditure on average, but there is lots of variation on the theme of average. People who generate more heat from calories have fewer available with which to make fat. They tend to be people who can eat a bit more, and stay thin anyway.

But that's a drop in the bucket compared to resting energy expenditure. Roughly 65 percent of calories are burned to support the fundamental workings of cells and organs that keep us alive. The number of calories burned at rest, and the actual percentage of total calories burned this way, also vary substantially around the average. People with a high resting energy expenditure are, in our modern world of epidemic obesity, the fortunate few most people love to hate: the folks who cannot seem to get enough to eat, and can't put weight on when they try.

If you don't control your thermogenic tendencies, nor your resting energy expenditure — who does? The idiosyncrasies of the genetic hand you were dealt, which are not necessarily idiosyncratic at all.

Take the case of the Pima Indians, for instance.

When the Pimas live a traditional lifestyle and eat traditional Pima foods — mesquite and tepary beans, for example — they have unremarkable health. When they live and eat like everyone else in America, they develop almost universal, severe obesity and diabetes. For a time, the Pimas had the highest rates of obesity and diabetes on the planet, and they are still, alas, contenders for those laurels.

The dire plight of the Pimas resulted in intensive study of them, and it led to both revelations, and the obvious. The Pimas have, uniformly, a very low resting energy expenditure. They are, in other words, highly fuel efficient — even for a fuel efficient species — and it makes perfect sense. The Pimas lived for generations in a harsh desert climate where food was unusually scarce, and physical activity demands unusually high. Pimas who were not highly fuel efficient simply didn't survive long enough to pass on their disadvantage to any future generation of Pimas. (People who don't live to make babies make very poor ancestors.)

Presumably, the variations in metabolic efficiency to which we are all subject can also be traced to variations in the experiences of our ancestors. But for most of us, the magnitude of genetic mixing that has gone on makes those pathways impossible to follow.

So we wind up feeling — those among us who gain weight easily and lose it with extreme difficulty, if at all — that we are enigmas, outcasts, anomalies. We feel that the claim that we can eat less, exercise more, and weigh more nonetheless falls on unsympathetic ears.

Mine are not such ears. I hear you, and I believe you. It is about calories in versus calories out, but the calories required to reach or exceed a personal threshold vary widely, and under influences we neither choose nor control. That isn't fair, but who every promised it would be?

As a practical response to this, I am considering the establishment of a national exchange for the weight loss resistant, so these stories come together and reach a critical volume. I would also like to use it as a means of troubleshooting the problem; many heads will be better than any one. Let me know via email if you would like to join us.

In the interim, I remain unwilling to refute Sir Isaac. But that we are, despite being much the same, quite a bit different — I fully appreciate. I bet Newton would agree; I hope everyone does.

Was everyone else startled when they first saw Kelly Osbourne at the Emmys on Sunday? She has lost quite a lot of weight. So much weight she pretty much looks like a different person. Not all of it is the weight though – she did away with the lavender hair, and now she’s doing some strange ashy blonde that ages her terribly. Anyway, Kelly took her new body to the Pussycat Dolls show last night, after tweeting the above photo. Kelly performed with Carmen Electra and Mya, and Kelly’s mom showed up with her new face to watch the performance.

Can I just say something? I’m a little disappointed in Kelly. I want her to be happy and healthy, but I think she’s just spinning to a bad place, honestly. Her weight loss is drastic, and she just recently broke it off with her fiancé, who broke her heart after probably-definitely cheating on her. This whole thing strikes me as “I’m doing fine on my own” move, and it’s not really coming across as “over it.”

Plus, she just looks extremely trashy in these getups. And not in a “trashy in a sexy way.” Poor Kelly.

On the left is Drew Carey back in January of this year, and on the other side is Larry King Drew last night at a CBS event in Beverly Hills. Drew tells UsWeekly that he pulled some John Goodman shit by losing almost 70lbs in just a few months. Drew also said that once he cuts the excess skin off his body, he will throw it in the blender with some Crayons and make it into liquid eyeshadow to spread on Mimi's beautiful lids.

The National Enquirer reported a few months ago that Drew was trying to lose weight by injecting himself appetite-suppressants, but he claims he dropped the fat by watching this video on a loop. No, he said he lost it the Star Jones way: DIET & EXERCISE! Drew said that thanks to a cardio program, he is now diabetes free! Drew said, “My fiancee has a 5-year old, and wow, I'd love to see him graduate. I'd love to be able play with him without getting tired, enjoy my life and watch him grow.

Who knew that when you peeled several layers off of Drew Carey, you'd find Pat O'Brien's long lost brother underneath! Seriously, I'm almost expecting Drew to ask me if I want to get some coke and hire some strippers. The answer is yes, by the way.

Jared Gaither finally cleared the air on why he had missed 2½ days of practice earlier in the week, and the offensive tackle chalked it up to nothing more than overworking a body that is getting accustomed to the rigors of training camp.

“I had a couple cramps,” he said after Tuesday’s morning session. “I dropped a couple pounds over the summer just preparing for a great season. I lost a little bit more than I was anticipating and cramped up a little bit my first time out there. I’m back now, and I’m fine.”

As has been documented, the 6-foot-9 Gaither has shed 29 pounds from his previous playing weight of 340 pounds. Gaither underwent some blood tests on Monday, but the results were negative.

Gaither also discussed his weight loss, which has emerged as an issue. The noticeably slimmer Gaither had said that he dropped the weight to alleviate any undue stress on foot and toe injuries that had plagued him last season.

But coach John Harbaugh sounded somewhat displeased about Gaither’s transformation, and Gaither revealed that he had not broached the coaches about losing weight during the offseason.

“No, we had not previously talked about it,” he said. “I wasn’t planning on losing this much weight, but that’s done now and we’re looking forward to put it back on.”

Gaither added, “To me, the weight, that’s not a big problem. I’m focused on playing football and making sure that I’m 100 percent. Coach Harbaugh, anything he has to say, I’ll abide by it and continue to work.”

Gaither said he has had conversations with the coaches about adding some weight, but he wouldn’t specify how many pounds would be sufficient. “We talked about it,” he said mysteriously.

Finally, Gaither dismissed speculation that the weight loss would make him better suited at left tackle rather than right tackle — the positions that he has been rotating at with Michael Oher. “Whatever position I play, my weight will be fine,” he said.

The first thing you'll want to do is get fantastically drunk; Exceptionally so. Make sure to keep an eye on the critter as the internal heat will begin to blister the body and create oily fissures. Catch as much of the delicious spurting hot fat as you can in a metal cup, delicious as a hearty aperitif, shared family style, before the upcoming main course.

Post grease cup, you are ready to serve. Take down the wire, rub with leaves to remove remnants of fur, and use your trusty hunting knife to cut lengthwise across the marmot starting at its rear end and extending to its throat. As you butterfly the body, you will notice that if you've bashed the thing correctly, a rewarding bouquet of steaming guts , coupled with strands of stringy marmot meat await. Serve al fresco on the forest floor and eat with your hands, making sure to ease each morsel down with a healthy swig of arkhi.

Oh, and one last thing. If you happen to be enjoying this delicacy with Mongolian huntsmen, do try your best to hold it all in, as otherwise you will appear both rude and awkward.

Lately, my job has me examining numerous blogs on excess weight loss and far better well being. A single day following reading through the blogs, a neighbor saw my health and fitness image in my husband's wallet. He wanted to know why my husband was carrying around a image of another woman. I received altered that considerably! I had been unrecognizable to a single of our closest neighbors! That inspired me to take up the challenge and get myself into condition. This really is my story.

A Bit of Background Data

Back again in my 20s, I made the decision to acquire into shape. I workout in the mornings (4:30 to become exact) and ate a healthful diet. I read all the conditioning magazines and boy did I look outstanding. Then anything changed.

Like a lot of ladies, children arrived along. My rock solid 115 pound 5 foot self waned away. In its location arrived a 135 pound blob. Now 15 lbs does not sound like a ton and for years, I denied it myself. However, I lost muscle body weight and replaced it with weight. Consequently, I went from 12% system weight to some whopping 30% body fat.

I married my husband weighing in at 95 lbs. Yes, I am that short. At 34, I had been 40 pounds heavier than on the day I got married and that was depressing.

Starting to Consume Healthful

When I determined to adjust, I couldn't for that life of me keep in mind what I ate to acquire into that rock hard entire body. I racked my brain and could not think of something. I vaguely remember protein bars to include muscle but which is it.

So, I turned to my blogs. Consistently, the blogs have been saying to eat natural foods. It's like going vegetarian but much more eating things as near to nature as feasible. This was fine with me; I don't like pre-packaged foods anyway.

I started consuming right but I nevertheless was exhausted from shopping with my girls. So, I made a decision I acquired to include the dreaded physical exercise.

Ok, so now I've to mention I was diagnosed with breast cancer at age 32. I've not exercised because. This was a scary proposition for me and I guess following two many years, my cancer excuse is rather worn out.

This is when I bring up walking. I made the decision to walk one mile a evening. There happens to become a one mile track around my home, so I don't need to do mindless laps close to a track.

The first evening, I felt like I had been heading to die. It didn't appear such as the best concept but I kept going back. Then somewhere around the third week, I discovered myself pondering this wasn't so poor. It was obtaining easier. I even added an additional lap!

Touted as the hottest all-natural weight loss supplement for women, Curvelle is getting some positive press for its ingredients which are said to help control appetite and burn excess fat. But is this just another “too good to be true” magic bullet for the desperate? Should the buyer beware? Examine the claims and independent review and decide for yourself.

According to the manufacturer's Dec. 15 press release, Curvelle has seen a spike in the number of women rushing out to buy this all-natural weight loss supplement designed specifically for women. Early in December, an issue of First, a women's magazine, gave positive press to Curvelle, which is believed to be responsible for the increase in sales. In First's article entitled “Breakthrough FAT BLOCKER,” the magazine says it searched the Web, polled women and consulted with doctors to find a natural alternative to Alli, another over-the-counter weight loss drug. Alli was said in the magazine to cause gas and diarrhea if the user ate more than 15 grams of fat in a single meal.

iSatori Technologies makes Curvelle. CEO Stephen Adele says that “Curvelle is different, which is why it's getting such positive press. It contains only all-natural ingredients that have been shown to help control appetite and burn excess calories for energy, along with quality antioxidants to help you look great and feel healthy and energetic.”

The First article says Curvelle contains a blend of nutrients – red raspberry ketones, elderberry extract, cinnamon and green tea – that combats fat, gets rid of fatigue and increases metabolism. The raspberry ketones reportedly inhibit fat formation, and elderberry extract stimulates fat cells that leads to calming cravings and speeding up metabolism. Cinnamon lowers blood sugar, and green tea is said to target belly fat.

First featured 36-year-old Julia Lacy-Adele as a testimony to Curvelle's legitimacy as a weight loss supplement. Lacy-Adele's relationship, if any, to iSatori Technologies CEO Stephen Adele is unclear. However, Lacy-Adelle, says that after “having three kids, I was left with a mound of fat on my belly and hips. I was tired of dieting and trying to lose weight in such an unhealthy way, like I did when I was 20-something.” She went on a calorie-reduced diet, participated in regular exercise and took Curvelle. Since losing 43 pounds, she says that “now I love my body, feel healthier, and look years younger.” First contains other favorable testimonies as well. However, the reader is left to wonder if reducing calories and increasing exercise – without the use of Curvelle – would've resulted in similar weight losses.

According to a review of Curvelle on the WeKnowDiets website, the all-natural herbal extracts found in the Curvelle are common to weight loss products already on the market – despite CEO Adele's claim that Curvelle is “different.” Also, this product is like many others in that it also contains caffeine.

Adele's response to the question of whether Curvelle is really better than other over-the-counter weight loss drugs is that “women will have to decide that for themselves. But I have to admit that women love Curvelle, and they are buying it three bottles at a time. We've had an extremely high satisfaction rate because it works, extremely fast. And it doesn't make you feel jittery and cranky. It's very safe and gentle and doesn't come with nasty or embarrassing side effects. I think that may be why the editors at First chose to feature Curvelle. Of course, it's impossible to say exactly how much body fat each person will lose from using Curvelle. As individuals differ in their diet and exercise habits, so will results. Still, before too long, you should notice an improvement in your overall health markers, a reduction in your cravings for sugars and other 'diet-sabotaging' foods, which makes it the perfect supplement during the holidays; and you'll lose inches from your hips, waist, thighs, and anywhere you have stubborn body fat or unwanted 'bulges.' Based on the feedback we've received so far, we've found that Curvelle really is different from anything most women have ever tried before. And that's exciting for us to hear.”

However, WeKnowDiets concluded that Curvelle “is a basic formula” that “doesn't appear to be spectacular in any way.” The website also points out that the manufacturer doesn't offer a free trial, which is offered with other herbal appetite suppressants that are also caffeine-free. Bottom line, says the website, Curvelle “seems like it may not be worth a customer's time or money.”

But now that we’ve all had a good laugh, Kuczynski addresses a more serious diaper-related matter: the dark side of dressing one’s toddler in adult-inspired clothing, which contains sinister allusions to obesity and unemployment, apparently.

The line between adulthood and infancy continues to blur, perhaps because of our national rates of obesity. People swollen with fat look like giant babies, the lines and wrinkles pressed from their faces. Or, perhaps it’s the recession, which has uprooted millions of Americans from their jobs, leaving men and women in their 30s and 40s unemployed for years at a stretch. They may want to look at infants as young, capable adults, full of the promise and ambition they will never be allowed to exercise.

And don’t even get her started on those Three Dots T-shirts with extra-long sleeves! There’s nothing like the puddling of excess fabric around our wrists to catapult us back to infantile helplessness.

Everybody in the west knows the fabled effectiveness of weight-loss dietary programs designed around intense cardiovascular regimes which burn huge amounts of calories. The people who work hard, doing as much cardio as possible lose weight. Those that pump iron may get muscular, but they aren’t working as hard and they don’t lose weight, they gain it. The moral of the story? When it comes to losing weight, cardio works, weights don’t.

Yet life is more complex than that. In the real world, cardio workouts alone aren’t the failsafe way to weight loss. If you work too hard, you burn too many calories and your body eats your muscle which slows your metabolism down and makes you gain weight.

Brian’s tale

Let’s look at Brian. He’s a world class athlete. But he has been living high on the hog for quite a while. That wasn’t a problem until recently. Now, he is a bit bloated and fat and wants to go on a diet to lose weight to stay on his coach Hermann’s team. Three of Brian’s teammates, George, Jose Luis and Jose, are also in jeopardy of being cut from the team and need to lose weight. What should Brian do?

Well, Brian consults his sister Angela. She’s also on the team. While she has had a minor weight problem for the last ten years, she’s definitely in better shape than Brian right now. She tells Brian that she has dieted in the past and that the sure fire way to losing weight is working hard and burning a lot of calories in an intensive cardio program. Burning off calories has always worked for her.

Brian’s trainer, Dominque (DSK for short) tells him Angela is right and prescribes a tough cardio regimen for our friend. Brian follows the program assiduously. But rather than losing weight, he gains weight. What happened?

Does Jose Luis learn from Brian?

Jose Luis sees Brian’s experience and is petrified. His fitness coach DSK gave him the same advice. Now he’s not so sure this is the right thing to do. Looking around for answers, he asks his strength coach Rob P for advice. Rob P says "you’re working too hard and burning too many calories. Your body is eating into your muscle, which is slowing your metabolism down. And you aren’t strength training." Jose Luis is perplexed. "Wow I didn’t realize that you could burn too many calories. Rob P answers, "Let me explain how the body works. Your body needs a certain number of calories throughout the day in order to function, and more importantly for you, to maintain muscle."

Jose Luis cuts in with, “I don’t care about my muscle. What does that have to do with my losing weight?”

"Jose Luis, your muscle is going to be what regulates your metabolism. The more muscle you have, the more calories you will burn throughout the day. This is known as your base metabolic rate. If you burn too many calories by over-training, your body starts eating your muscles as well as your fat, slowing your metabolism. The result is you lose strength, you end up gaining weight and you become weaker."

"Now I get it. But I’m not sure if I can do that. If people see me doing strength training and not cutting back when everyone thinks eating a lot less is the only way to lose weight, I’m going to be off the team in a hurry?"

"Good point. I don’t know what to say.  Maybe you’re on the wrong team. Maybe you need to go solo, Jose Luis."

"Maybe."

Barry takes notes

Barry is not on Angela, Jose Luis and Brian’s team. He’s solo. So he doesn’t have the same constraints where eating less is the only way to lose weight. He has  a lot more options than George, Jose and company. If he chooses, he could try strength training too. If he does just enough cardio and just enough strength training, he might be able to lose the weight without cutting into his muscle too much. He would be stronger and his metabolism would be faster. When he went up against Team Euro, he would win hands down.

But Barry is advised by Larry and Tim who studied physical fitness at the same school DSK did. And their recommendation to Barry is that he follow a similar high intensity cardio but muscle-losing program that failed for Brian and that Jose Luis is bent on trying tempered with a bit of strength training.  Unfortunately, the cardio is too much and in all likelihood, Barry’s metabolism will slow. He will gain weight but also lose muscle.  Who cares, though? It’s not like Angela and Jose Luis are gaining muscle.  It only matters to Barry’s family who depend on how well he does to earn a living. And right now, it looks like they are looking pretty depressed.

Deficits, Spending and Taxes

Think of this as a tale of government spending and deficits. The longer-term goal is to reduce deficits as one would reduce weight. There are two ways to go about this. One is to cut as much spending as possible, the equivalent of a highly focused cardio regimen and its calorie burning intensity. The second is for the government to create jobs to increase GDP and the tax base while strengthening the competitiveness of the economy as one might pump iron to gain strength and muscle, increasing the metabolic rate. Clearly, if one over-trains by pumping iron and eating too much, one gains weight just as wasteful spending misallocates resources and increases the deficit. But, there are risks to cardio over-training which causes muscles to atrophy, slowing the metabolism and increasing weight where the economic analogue comes in the form of depression, the dead-weight economic loss of healthy companies’ insolvency, tax loss from high long-term unemployment, and ultimately higher deficits.

It looks like we’re in for a lot of cardio across the entire developed world and not enough strength training.  My prediction is serious weight gain and serious muscle loss. I now believe GDII is coming and I will have a lot more to say about this in an upcoming post.
